Adult Forms
The Georgia Indigent Defense Council and the Bridging
the Gap Translation Project developed several multi-lingual
forms for use with indigent cases.

The
Affidavit
of Indigence is the one shown in the Georgia Indigent Defense
Council's "Guidelines the Operation of Local Indigent
Defense Programs" and has been approved by the Supreme
Court of Georgia. It details a defendant's income
and expenses, assets and liabilities.
The Introduction
is an election to proceed without an interpreter, or alternatively
a request by the the defendant that an interpreter by appointed
to assist him or her.
The Answers
file is a list of expected answers to questions posed in
court. The English translation of each answer is also
shown to facilitate the court's understanding of the defendant's
response.
The Waiver
of Counsel and Entry of Plea
forms were developed by the Southern Center for Human Rights for use in
the settlement of the federal lawsuit against Sumter County.
The Waiver of Counsel form details the rights that a defendant
will give up if he or she wishes to proceed without the
assistance of an attorney. The Entry of Plea form
(originally created for use in DeKalb State Court) explains
not only the rights given up by a defendant entering a plea
of guilty or nolo contendere, but also forms the basis for
the plea itself.
In addition
to providing these forms "online" the Georgia Indigent
Defense Council has distributed printed copies of all noted
forms to each Superior Court Judge, and copies of the Affidavits
of Indigence to each Sheriff in Georgia.
